Chain of Custody>

Katahdin requires that a chain of custody (COC) be utilized for transmittal of samples to the laboratory. Normally, Katahdin provides a COC with bottle orders. As an option, below are links to electronic versions of the COC's. The Environmental COC's are in either Portable Document Format (PDF) or Excel (xls) format. The excel format will allow a client to add text to the COC electronically.

For Food/Microbiology projects the COC's are in either PDF or Word document format. The Word format allows client entry of text.

All services provided by Katahdin Analytical Services, Inc. are governed by our standard terms and conditions except where a signed contractual agreement exists.
